Description

Offers patient education and monitoring of diet and kidney function, with an emphasis on patient self management skills. Kidney care centres promote early identification of renal disease and provides preventative care and education to delay or avoid a future need for dialysis. Also provides support to patients during transition into hemodialysis, peritoneal dialysis or transplant. Services include access to a nephrologist, nurse, social worker and dietitian. Upon receiving a nephrologist referral, the centre mails a registration package to the client to complete and bring to the first appointment. Valid CareCard and all current medications should be brought to the first appointment.

Multidisciplinary team monitors patients with kidney disease. Also teaches self-management skills to slow progression of kidney disease and delay dialysis. Located on the fourth floor of the Fraser Wing.
