Provides in-home support services to those with chronic health conditions, disabilities or terminal illness so they can continue to live at home safely and independently.

Trained community health workers offer assistance with daily living activities such as bathing, dressing and grooming, as well as provide special exercises; caregiver rest and relief, and medication administration.

Eligibility: Adults aged 19 and over with health issues, age-related limitations or other disabilities. Clients must meet eligibility requirements to receive services; see website for detailed eligibility requirements.

Application Process: Call the Home Care Access Line for information or to request service. Current Home Health clients should contact their local office for questions regarding their enrolled services.
Program Fees: There may be a daily charge depending on client income.
